**Expense Claim Information**

All Network sponsored individuals who incur eligible travel expenses while at the conference will be required to complete and submit a Travel Expense Claim form. For a listing of expense policies related to the conference, please refer to the Guidelines for Registration, Travel Arrangements & Expenses:

Per Diem rates are as follows:

• Mileage = $.30/km or $.48/mi
• Breakfast = $8.00/day
• Lunch = $12.00/day
• Dinner = $20.00/day

You will find a copy of the Travel Expense Claim Form at:

You will notice in the top right hand corner of the Claim Form a return address is given for The University of Western Ontario, DO NOT submit your claims to that address.

All claims must be returned to the following address for approval:
